If you have formed a civil partnership and require a new civil partnership certificate, you can apply for this at the registry office. Please contact the registry office in whose jurisdiction the partnership was established. The civil partnership certificate proves that you have entered into a civil partnership with your civil partner. Among other things, it contains the names you use in the civil partnership. It also states the place and date of establishment. You can receive the civil partnership certificate in various forms:
