Procedure details

The occurrence of any of the following events must be reported immediately to the competent authority:

  • Emergency
  • Incident
  • Other significant incident
  • Loss of radioactive substances
  • Discovery and acquisition of radioactive substances
  • Contamination of metal through melting or other metallurgical processing of a radiation source

The report must contain all available information required for the assessment of the incident. As far as possible, the causes and effects as well as the measures taken to remedy the effects and prevent such an incident must be stated.

Responsibilities

  • The trade supervisory offices of the governments are responsible for X-ray equipment or interfering radiators.
  • Otherwise, the State Office for the Environment (LfU) is responsible.

The notification should be submitted by a person with expertise in radiation protection.

The report must contain all available information required for the assessment by the authority, e.g:

  • Category and reporting criteria of the incident according to Annex 14 or 15 of the StrlSchV
  • Type of application, medical procedure used
  • Date/period of the incident
  • Details of the operator/institution and functional area
  • Description of the event
  • Information on dose, other radiological specifications (nuclide, activity, etc.)
  • Information on the number of persons affected and information on gender and age
  • Description of the cause, consequences (health consequences, short-term and long-term consequences)
  • Details of the type of device and system
  • Details of the appliance manufacturer
  • Details of immediate measures taken (medical, operational)

The notification must be submitted to the local trade supervisory office or the Bavarian State Office for the Environment (LfU).

The respective authority records, checks and evaluates the notification and then forwards it to the Bavarian State Ministry of the Environment and Consumer Protection.

In the case of exposures in the context of medical and non-medical applications on humans, the competent authority also forwards the notification to the Federal Office for Radiation Protection (BfS) via the BeVoMed database.
